1. Concentration Shape at Annexure-I shall be downloaded from our website: www.orissamining.com & to be filled-up by the candidate in English including carefully conception the eligibility criteria prescribed for each post.
2. The applicant should pin recent colour passport mass photograph at top right feature of the concentration shape.
3. The applicant should furnish attested imitation of characteristic sheets & certificates of HSC or corresponding examination & onwards alongwith certificates towards age, caste, experience, etc.
